بررسی مشکلات احتباس ادرار در ام-اس - sooorooosh - 2012/12/26

مشکلات احتباس ادرار به صورت تکرر ادرار و عدم توانایی کنترل ادراردر بیمارای ام اس نمایان میشود.این معضل تاثیری نا خواسته در فعالیتهای اجتماعی فرد میگذارد.مشکلات مثانه عموماً ارتباطی مستقیم با ناتوانی و اسیب دیدگی قسمت تحتانی کانال نخاعی سیستم اعصاب مرکزی دارند.
مشکلات مثانه در بیماری ام اس عموماً به صورت تواتر در دفع ادرار و لزوم فوری برای دفع ادرار مشخص می شوند.
مشکلات دیگری نیز مانند در مشکل در شروع ادرار کردن، تردید، نشت و
احساس ادرار کردن ناقص نیز وجود دارند.که نسبت به دو مشکل رایج بیان شده((تواتر د فع ادرار و لزوم فوری دفع ادرار)) نادر تر میباشد.
هنگامی که احتباس ادرار رخ میدهد احتمال بروز عفونت های مثانه در بیمار نسبت به افراد عادی بیشتر میشود.که پزشکان باید این مطلب را مد نظر داشته باشند .
قسمتهای مختلفی در مغز وجود دارند که قسمتهای دخیل در دفع ادرار هستند. به نسبت آنکه چه قسمتی از این سیستم کنترلی در بیماری ام اس آسیب می بیند احتمال بروز هر کدام از علائم ذکر شده در دستگاه دفع ادرار وجود دارد
اهداف درمانی برای رفع مشکلات مثانه در سه گروه تقسیم بندی میشود:
A-کاهش علائم اختلال ادراری
B-درمان عفونت های ادراری
C- کاهش عوامل اخلال گر در عملکر کلیه ها
روش های درمانی را میتوان در دو گروه اصلی تقسیم بندی کرد:
1-درمانهای دارویی
2- درمانهای غیر دارویی
a-بررسی درمان های دارویی :این نوع از درمان ها ارتباطی مستقیم با نوع اختلال ایجاد شده در عملکرد دستگاه مستانه دارد. و با توجه به نوع اختلال داروی خاص برای رفع آن و یا کاهش شدت آن تجویز میشود.
به عنوان مثال داروی آلفازوسینalfuzosin برای مشکلات عدم توانایی نگهداری ادرار تجویز میشود.یا داروی تروسپیم trospium و فِلِیوکسِت flavoxate برای کنترل ضرورت و بیاختیاری در دفع ادرار مصرف میشود.
همچین در صورت بروز مشکلات شب ادراری در بیمار از داروی دسموپرسین
desmopressin استفاده میکنند
b-بررسی روشهای غیر دارویی:تمرین و کار بر روی عضلات کف لگن -باز آموزی عملکرد مثانه-و تناوب در کاتتریزاسیونcatheterization
کاتتر یزاسیون:روش پاک سازی مجاری ادراری برای بهبود عملکر دستگاه دفع ادرار با جسمی سوزنی مانند
اطلاعات آماری بروز مشکلات مثانه در بیماران ام اس:
در مراجع مختلف آمارهای متفاوتی با توجه به محل زندگی بیمار و نوع مذکذ و مونث بودن بیمار برای بروز مشکلات مثانه ارئه میشود
معمولاً مشکلات مثانه در 70 تا 80 درصد بیماران مبتلا به ام اس به وجود می آید
این در حالی است که معمولاً بین 50-90 در صد از بیماران با این مشکلات را حداقل یک مرتبه تجربه میکنند

دوستان داروی مورد استفاده تو ایران اسمش اکسی بوتینین هست اسمش تو این لینکها نبود نمیدونم چرا...ولی دارویی هست که پزشکا ازش برای کنترل تککرر ادرار استفاده میکنن
اکسی بوتینین کلراید:
اکسی بوتینین کلراید (به انگلیسی: Oxybutynin Chloride)‏

نامهای تجارتی : Ditropan - Exybutin

رده درمانی: آنتی اسپاسمودیک مجاری ادراری

رده فارماکولوژیک: نسل سوم آمین سنتتیک

اشکال دارویی: قرص ۵ میلی گرمی
مکانیسم اثر آنتی اسپاسمودیک اکسی بوتینین از طریق افزایش ظرفیت مثانه و کاهش فرکانس انقباضات عضله دترسور اعمال می‌شود. این دارو، دارای اثرات آنتی موسکارینیک بر روی عضلهٔ صاف می‌باشد.
تنها مورد مصرف این دارو، بعنوان آنتی اسپاسمودیک در مثانه نوروژنیک می‌باشد.
طریقه مصرف:

دربالغین: ۵ میلی گرم از راه خوراکی ، ۲ تا ۳ بار در روز تجویز می‌شود. حداکثر دوزاژ دارو ۵ میلی گرم چهار بار در روز است.

در کودکان بزرگتر از ۵ سال : ۵ میلی گرم از راه خوراکی دو بار در روز تجویز می‌شود.حداکثر دوزاژ دارو ۵ میلی گرم، سه بار در روز است
تداخلات دارویی:

اثرات آنتی کولینرژیک این دارو، به دنبال مصرف سایر داروهای آنتی کولینرژیک، نظیر داروهای ضد افسردگی، فنوتیازینها و هالوپریدول تشدید می‌گردد.
اثرات تضعیف کنندگی این دارو بر روی دستگاه عصبی مرکزی به دنبال مصرف سایر داروهای تضعیف کننده سیستم اعصاب مرکزی نظیر : آنتی هیستامینها، نارکوتیکها و داروهای آرامبخش و خواب آور تشدید می‌گردد.
عوارض جانبی:

در دستگاه اعصاب مرکزی: سرگیجه، بی خوابی، خواب آلودگی، برافروختگی
در دستگاه قلب و عروق: طپش قلب، تاکی کاردی
در دستگاه تناسلی ادراری: ایمپوتانس (ناتوانی جنسی)، احتباس ادراری، اشکال در ادرار کردن
در پوست: کهیر، راش، واکنش‌های شدید آلرژیک در بیماران حساس به آنتی کولینرژیکها.
در دستگاه گوارش: تهوع، استفراغ، یبوست
در سایر دستگاهها: کاهش تعریق، تب، مهار ترشح شیر

توجه مهم: در صورت ایجاد واکنش افزایش حساسیت و یا علایم دیسترس تنفسی یا اشکالات عروقی و یا بروز تحریک در سیستم اعصاب مرکزی، باید مصرف اکسی بوتینین را فورا قطع نمود.

من الان نزدیکه 1 ساله این مشکلو دارم نمیدونستم به ام اس مربوطهangry3
شرمندم از گفتن این حرف ولی مثلا صب که میخوام برم بیرون میرم wc بعد شب مثلا ساعت 10 یادم میوفته من صب رفتم wc و از صبه نرفتمangry3
نمیدونستم از ام اسه....حالا این بده یعنی؟ضرر داره یعنی؟میخوام بیخیالش شم و هویجوری ادامه بدم به زندگیevilgrin0039.gif

شیرین یعنی واقعا نمیدونستی؟
اینجوری کلیه هات داغون میشن.
احتباس یا تکرر ادرار از مشکلات ام اسه

من تا یکی دو سال پیش نمیدونستم برای کنترل ادرار دارو هست و برای اون مشکلم دارویی وجود داره.

تو که تکرر ادرار نداری برای این مشکلت نمیدونم دارویی هست یا نه بهتره به دکترت بگی

سعی کن مصرف مایعاتت رو بیشتر کنی.شاید کم آب میخوری که کم ادرار داری

قبول که باید بیخیال بشیم اما نمیشه به سلامتیمون و مشکلاتمون توجه نکنیم.

بده که انقدر کم میری توالت

من که قبل بیماریم هم همیشه زود به زود میرفتم دستشویی مخصوصا اگه مایعات بخورم

اصلاً کاری به ام اس نداره این شیرین خانم...من تمام مشکلاتی رو که در ام اس به وجود میاد و شایع هست رو نوشتم...
ممکن باعث افزایش تواتر در تعداد دفعات بشه اما باعث کم شدن نمیشه...
هیچ مشکلی ندارین...ایشالا هیچ وقت هم به وجود نمیاد...ولی هیچ جا رو هیچ منبعی در مورد افزایش بازه زمانی بین دفعات دفع چیزی گفته نشده...یعنی کتاب رو هم نگاه کردم در این مورد به چیزی اشاره نکردهconfused
در ضمن من ریپورت ام ار ای شما رو دیدم ...شما اصلاً تحریک و ضایعه نخاعی نداری که بخواد در انتهای کانال نخاعی باشه ....در اون صورت بر عملکرد سیستم دفع ادرار تاثیر میزاره

اما من دوست دیگری مو هم میشناسم که احتباس ادرار داشت
الان گاهی زود به زود نیاز به توالت رفتن پیدا میکنه
یعنی ممکنه این احتباس رفع و حتی برعکس بشه
البته دیر به دیر ادرار داشتن فرق میکنه با احتباس ادرار
اما خب دیر به دیر توالت رفتن مسلما برای کلیه ضرر داره
