ام اس کشنده نیست محدود کننده است

MS (مولتی پل اسكلروزیس) یا بیماری ماه رویان حمله سیستم ایمنی بدن به خود بدن با علت ناشناخته است. این بیماری سلسله اعصاب مركزی (مغز و نخاع) را در قسمت های مختلف درگیر می كند. به نحوی كه میلین پوشش رشته های عصبی مورد حمله گلبول های سفید بدن فرد قرار می گیرد.

البته تحقیقات جدید حمله گلبول های سفید به رشته های اكسونی را نیز نشان داده است. طبق آمارها در دنیا از جمله ایران بیماری MS در حال افزایش است به نحوی كه هم اكنون سی هزار بیمار مبتلا به MS در ایران وجود دارد.

MS از بیماری هایی است كه در اكثر موارد جوانان، قشر فرهیخته و تحصیلكرده جامعه را درگیر می كند. این مسئله ما را با این سئوال روبه رو كرده است كه چرا MS از لحاظ اقتصادی در طبقات پائین اجتماع كمتر مشاهده می شود. هنوز پاسخی به این سئوال داده نشده است.
علل بیماری

عوامل موثر در ایجاد بیماری MS به طور كامل شناخته شده نیست. بنابر اظهارات رئیس انجمن MS ایران بررسی ها نقش پنج عامل را در بروز این بیماری موثر دانسته اند.

ژنتیك، پراكندگی جغرافیایی و محیط (نظیر آلاینده های زندگی شهرنشینی)، استرس، عوامل ایمونولوژیكی، میكروب ها و ویروس (نظیر ویروس تبخال تیپ یك HSV۱) كه در میان عوامل ذكر شده نقش ژنتیك شناخته شده تر است.
گفته می شود در هشتاد درصد مبتلایان به MS خویشاوندان نزدیك MS ندارند با این وجود در بعضی از خانواده ها بیشتر دیده می شود. وجود زمینه بیماری و استعداد خانوادگی به عنوان عواملی موثر در افزایش احتمال ابتلا در خویشاوندان نزدیك مطرح می شود.

از طرفی طبق برخی تحقیقات در صورت درگیری یكی از قل ها در دوقلویی تك تخمكی (هموزیگوت) الزاماً قل دیگر درگیر نیست. بنابراین نمی توان صرفاً ژنتیك را در این مسئله دخیل دانست.

«با دور شدن از خط استوا این بیماری شیوع بیشتری پیدا می كند به نحوی كه در مناطق سردخیز با درصد بیشتری از بیماری روبه رو می شویم.

یكی از دلایل این مسئله را كمبود ویتامین D در مناطق سردخیز ذكر می كنند كه قطعیت آن به اثبات نرسیده است. مثلاً در شمال اروپا شایع تر از جنوب اروپا است.

در كشورهای آسیایی نظیر ژاپن، هند و خاور دور بروز MS كمتر بوده و برعكس در ایران روبه افزایش است. هنوز پاسخی برای این پراكندگی های متفاوت داده نشده است.»

مبتلایان به MS به دلیل اختلالات سیستم ایمنی در برابر عفونت ها (میكروبی یا ویروسی) آسیب پذیرتر هستند. اما مشخص نیست در بروز این بیماری بین آلودگی های ویروسی و غیرویروسی كدام یك نقش مهم تری دارد.

طبق یك مطالعه ای كه اخیراً انجام شده عدم تماس با ویروس تب خال تیپ یك (HSV۱) به عنوان عامل خطری در بروز بیماری MS مطرح شده است.

اما به طور كلی هنوز نقش ویروس ها در بروز و سیر بیماری مشخص نیست و همچنان تحقیقات بر روی نقش عامل های ویروسی و غیرویروسی (نظیر كلامیریا) ادامه دارد.»

برخی افراد در استفاده از واكسن تردید دارند و از آنجا كه واكسن ها را با عوامل ایجاد بیماری ها به صورت مرده، ضعیف شده و... تهیه می كنند بنابراین تحقیقاتی در رابطه با تاثیر یا عدم تاثیر واكسن بر روند بیماری MS در حال انجام است. هنوز نتایج قطعی به دست نیامده است. بنابراین تا مشخص شدن نتایج پیشنهاد می شود واكسیناسیون تحت نظر پزشك متخصص انجام شود.
