تهران - روانپزشک مرکز تحقیقات ˈام اس ˈ بیمارستان سینا گفت : ˈام اسˈ یک بیماری مزمن سیستم ایمنی است ..این بیماری در هر سنی ایجاد می شود ولی معمولا در سنین 40-20 سال تشخیص داده می شود .

به گزارش انجمن ام اس ایران ،دکتر ˈفرناز اعتصامˈ افزود:با توجه به اینکه درگیری سلول های عصبی در هر قسمتی از مغز و طناب نخاعی می تواند اتفاق بیفتد ، طیف وسیعی از علائم را می تواند در بر داشته باشد .

به گفته وی این علایم اسپاسم ، ضعف عضلانی ، ناتوانی حرکتی، علائم چشمی مانند دوبینی،تاری دید یا از دست رفتن قدرت بینایی، اختلال عملکرد روده و مثانه بصورت بی اختیاری یا تکرر ادرار، یبوست یا نشت مدفوع، علائم حسی مثل کرختی ، بی حسی ، سوزن سوزن شدن و درد در نواحی مختلف بدن، مشکلات جنسی ، سرگیجه و اختلال تعادل، در بیماران مبتلا به MS مشاهده می شود

وی ادامه داد:از زمان شناخت ام اس به عنوان یک بیماری ، توجه پزشکان و بیماران اکثرا بر ناتوانی جسمی همراه با این بیماری متمرکز بوده است ولی در دهه اخیر ، اختلال در عملکرد حوزه های مختلف شناختی در بیماران ام اس مورد توجه بسیاری ازافراد قرار گرفته است .

این روانپزشک با بیان اینکه اختلال شناختی با شیوع 40 تا70 درصدی همراه است ،گفت: در همه زیرگروه ها و مراحل این بیماری و حتی اشکال خفیفتر آن مانند سندرم مجزای بالینی(CIS) و ام اس خوش خیم مشاهده می شود.

وی گفت :تظاهرات بالینی این اختلال ، متغیر است و می تواند از اشکال خفیف بصورت مشکلات در حافظه کوتاه مدت ، توجه و تمرکز و اختلال در عملکرد اجرایی تا بطور نادربصورت نقایص شدیدی مانند دمانس، دیده شود.

اعتصام اظهار داشت :بیماری ام اس، بیشتر افراد را در اولین سال های زندگی کاری درگیر می کند. اختلال شناختی در این افراد، می تواند کاملا ناتوان کننده بوده وهزینه سنگینی را از نظر فردی و اجتماعی وارد کند.

وی اضافه کرد :اختلال شناختی نه تنها دارای اثرات بالقوه ای برکاهش کیفیت زندگی بیماران است، بلکه می تواند بر عملکرد روزانه مانند قابلیت تصمیم گیری های لازم در روند درمان بیماری و پذیرش و پیگیری دستورات درمانی ، حفظ روابط اجتماعی و توانایی رانندگی تاثیرگذاشته و به عنوان عامل مهمی در از دست دادن شغل مشارکت نماید ، بنابراین تشخیص و درمان آن در اسرع وقت ضروری است .

این روانپزشک مرکز تحقیقات ام اس بیمارستان سینا در ادامه افزود: در حال حاضر از دو رویکرد درمان دارویی و غیر دارویی در مقابله با اختلال شناختی در افراد استفاده می گردد. هنوز درمان دارویی کاملا موثری برای درمان اختلال شناختی در بیماران ام اس وجود ندارد.

اعتصام خاطر نشان کرد :تحقیقات انجام شده این مسئله را مطرح می کنند که دارو های تعدیل کننده بیماری مانندˈاینتر فرونها ˈممکن است از پیشرفت این اختلال بکاهندو داروهای دیگری از قبیل مهارکننده های مانند دونپزیل و ریواستیگمین نیز در این زمینه بکار می روند اما تاکنون شواهد محکمی مبنی بر اثربخشی آنها در بدست نیامده است .

وی افزود: تاکنون مداخلات مختلفی برای بازتوانی ذهنی بیماران انجام شده .این راهکارها شامل تمرینات شناختی با یا بدون استفاده از برنامه های رایانه ای ،آموزش کاربرد وسایل وروش های کمک حافظه ، آموزش مهارت های ارتباطی، تمرینات فیزیکی و رویکرد های شناختی-رفتاری مانند آموزش روش های حل مسئله ، بهبود توانایی های انطباق و شیوه های خود مدیریتی به شمار می رود. رود .

وی گفت : بسیاری از برنامه های بازتوانی ذهنی در مراکز درمانی تحت نظر متخصصین صورت می گیرد لذا مشکلاتی مانند تعمیم مهارت های یادگیری شده ، میزان دسترسی همگانی به آنها، زمان محدود آموزش و یادگیری، و بهره وری هزینه همراه با اجرای این برنامه ها وجود دارد .

نهمین کنگره بین المللی ام اس ایران 11 تا 12 آبان 91 در هتل المپیک ایران برگزار می شود .
