بازدید : 3066 مرتبهتاریخ : شنبه 10 بهمن 1388پژوهشگران می گویند ژن درمانی از توان بالقوه معالجه آرتروز رماتیسمی و سایر بیماری های غیرمهلک برخوردار است.
دانشمندان طی یک رشته آزمایش نشان داده اند که امکان معرفی یک ژن تازه به بدن برای متوقف کردن التهاب در مفاصل متورم به شیوه ای ایمن وجود دارد.
تیمی از محققان دانشگاه پیتزبورگ سلول های اصلاح شده ژنتیکی را به بند انگشت (در ناحیه پنجه) 9 بیمار مبتلا به ورم مفاصل تزریق کردند که نتایج امیدوار کننده ای به دنبال داشت.





نتایج این مطالعه در نشریه اقدامات آکادمی ملی علوم منتشر شده است.
البته این یک آزمایش مقدماتی برای محک زدن این شیوه بود و روشن شدن این موضوع که آیا این تکنیک در عمل موثر خواهد بود یا خیر، نیازمند مطالعات بسیار بیشتر است.
سلول های اصلاح شده پس از یک هفته از بدن بیماران که جملگی زن بودند خارج شد تا برای جایگزین کردن مفاصل بند انگشت تحت عمل جراحی قرار گیرند.
با این حال این یافته ها ممکن است به رفع نگرانی پیرامون ایمن بودن ژن درمانی که در جریان سایر آزمایش ها مطرح شده بود کمک کند.

انتقال ویروس

تیم دانشگاه پیتزبورگ از یک ویروس فعال نشده برای معرفی ژن اصلاح شده به داخل سلول بیمار استفاده کرد.

همین تکنیک توسط تیمی از دانشمندان فرانسوی که در جستجوی راهی برای معالجه از کار افتادگی حاد سیستم ایمنی بدن بودند به کار گرفته شد.

اما سه کودک که در مطالعه فرانسوی ها شرکت کرده بودند متعاقبا به سرطان خون مبتلا شدند که باعث بروز تردیدهایی در مورد ایمنی این شیوه شد.

از این رو، تیم محققان پیتزبورگ بیماران را برای مدت پنج سال از نزدیک تحت نظر قرار داد، اما هیچ نشانه ای از عوارض منفی معالجه در آنها نیافتند.

البته آنها اذعان کردند که از یک ژن متفاوت استفاده کرده و در مقایسه با تیم دانشمندان فرانسوی هدف متفاوتی را دنبال می کرده اند، اما استدلال می کنند که یافته های آنها نشان می دهد که شیوه ژن درمانی برای آرتروز رماتیسمی تکنیکی ایمن است.

در آرتروز رماتیسمی، دو دسته از سلول های سیستم ایمنی بدن به نام مایکروفیج و لیمفوسایت، روکش مفاصل را اشغال می کنند.

این سلول ها، پروتئینی به نام سایتوکین تولید می کنند که به سلول های سینوویا که روکش مفاصل را تشکیل می دهد چسبیده و باعث ترشح ماده ای می شود که به التهاب می انجامد.

تیم محققان دانشگاه پیتزبورگ یک ژن را به گونه ای اصلاح کرد که از طریق جلوگیری از چسبیدن سایتوکین به سلول های سینوویا، توانایی بالقوه برای مسدود کردن این فرآیند را داشته باشد.

استخراج سلول ها

از آنجایی که ایمنی از نگرانی های عمده آنها بود، این ژن را به سلول هایی تزریق کردند که از بدن بیماران استخراج شده بود.

به علاوه این سلول ها چند هفته بعد و از پس از انجام آزمایش هایی برای مشخص شدن تاثیر این معالجه به مفاصل بیماران تزریق شد.

آزمایش ها نشان داد که خوشه های سلولی حاوی این ژن در سطح لایه سینوویا ظاهر شدند و میزان بسیار کمتری مواد التهاب آور تولید کردند.

پژوهشگران امیدوارند بتوانند طی آزمایش های تازه ژن های اصلاح شده را مستقیما به مفاصل متورم تزریق کنند.

السا بوسوورت، از انجمن ملی آرتروز رماتیسمی بریتانیا، گفت ژن درمانی تحولی "خیلی خیلی هیجان انگیز است."

با این حال او افزود که باید آگاه باشیم که دانشمندان فاصله بسیار زیادی با کشف یک درمان ایمن و قابل دسترسی برای بیماران دارند.

دکتر مادلین دوی، از گروه مبارزه تحقیقات آرتروز، گفت که فائق آمدن بر مشکلات مربوط به ایمنی ژن درمانی سال ها زمان برده است.

وی افزود: "این (تحقیقات پیتزبورگ) پیشرفت کاملا امیدوارکننده ای به نظر می رسد که خبر خیلی خوبی برای بیماران مبتلا به آرتروز است."
