به گزارش مطالعه ای جدید افزودن داروی daclizumab به درمان متداول ام.اس ( اینترفرون بتا) بیش ازدرمان منحصر با بتا اینترفرون در کاهش فعالیت بیماری ام اس موثراست .
دانشمندان در مطالعه غیر تصادفی قبلی متوجه شدند که daclizumab (آنتی بادی مونوکلونال انسانی ) موجب کاهش فعالیت های بیماری ام.اس می شود .
این مطالعه دو مرحله ای جدید در 51 مرکز در ایالات متحده ، کانادا ، آلمان ، ایتالیا و اسپانبا ،در 230 نفر با بیماری ام.اس پیشرونده که اینترفرون بتا دریافت می کردند انجام شد.. آنها به طور تصادفی گروه بندی شدند . یک گروه daclizumab را با دوز بالا دریافت کردند (2 میلی گرم / کیلو گرم ، هر دو هفته) ، گروه بعدی daclizumab را با دوز پایین(1 میلیگرم / کیلوگرم در هر چهار هفته) وگروه دیگر دارونما یا پلاسبو دریافت کردند . درمان ترکیبی به مدت 24 هفته ادامه داشت.
MRI مغزی بیماران هر چهار هفته یکبار در بین هفته 8 و 24 مطالعه و به منظور تعیین تعداد کنتراست گادولینیوم جدید و نوع پیشرفت ضایعات و مشخص شدن قسمتی که بیماری در آن فعال است گرفته شد .
در پایان مطالعه ،میانگین تعداد کنتراست گادولینیوم جدید و ضایعات پیشرفته در گروه با دوز بالا (75 مورد) ، 1.32 و در گروه با دوز کم (78 نفر) 3.58 و در گروه دارونما بود (77 نفر) 4.75 بود.
میزان سلول های طبیعی نابود کننده ی CD56bright در بیمارانی که daclizumab مصرف می کردند هفت الی هشت برابر بالاتر از بیمارانی بود که دارونما مصرف می کردند.
"این مطالعه تاییدی است جهت فراهم کردن شرایطی برای گسترش درمان با daclizumab برای افزایش سلول های از بین برنده ی CD56bright .
حمایت از این تئوری و گسترش سلول های طبیعی از بین برنده ی CD56bright ممکن است با مصرف daclizumab بطور غیر مستقیمی با کاهش فعالیت بیماری موجب کاهش برخی از ضایعات بیماری گردد . علاوه بر نتایج مطالعات پیشین در اثر بخشی daclizumab در ام.اس با از بین بردن سلولهای CD56bright ، شواهد دیگری نیز استفاده از یک تابع پتانسیل تنظیم سیستم ایمنی برای از بین بردن سلول های CD56bright را پیشنهاد می کند : آنها دربرخی از شرایط بطور طبیعی از ایمنی لازم و طبیعی برخوردارند ، از جمله ، بارداری و .....
با استتنتاج از این مطالعه می توان گفت:
"این تحقیق تصادفی کنترل شده نشان می دهد که مصرف daclizumab همگام با اینترفرون بتا می تواند ضایعات جدید را دربیماری ام.اس کاهش دهد در مقایسه با مصرف اینترفرون بتا - به تنهایی .
مطالعات بیشتری برای تعیین خطرات طولانی مدت و مزایای daclizumab لازم است .
این تحقیق در 15 فوریه در نشریه نورولوژی لانست منتشر شد
