تاندون آشیل بزرگترین و مهمترین تاندون عضلانی در بدن است – تاندون آشیل تاندون عضله سه سر ساقی است و در پائین به استخوان پاشنه پا اتصال دارد. التهاب و آسیب های آن علل مختلفی دارند که عموماً‌ ناشی از پرش, رقص پای مکرر, تمرین بیش از اندازه و گرم کردن نامناسب و کفش نامناسب و کف پای صاف است.
پیش زمینه

تاندون آشیل بزرگترین و مهمترین تاندون عضلانی در بدن است – تاندون آشیل تاندون عضله سه سر ساقی است و در پائین به استخوان پاشنه پا اتصال دارد. التهاب و آسیب های آن علل مختلفی دارند که عموماً‌ ناشی از پرش, رقص پای مکرر, تمرین بیش از اندازه و گرم کردن نامناسب و کفش نامناسب و کف پای صاف است


از التهاب و آسیب های تاندون آشیل آمار دقیقی در دست نیست چرا که بسیاری از آسیب های تاندون آشیل بدون مراجعه به پزشک بهبود می یابد اما تخمین زده می شود که شیوعی بین 18-5/6% در دوندگان داشته باشد.

آناتومی کاربردی
آشیل تاندون عضله سه سر ساقی است که در پشت ساق قرار دارد. این تاندون 15 سانتی متر طول دارد و در پائین با یک زاویه 30 درجه به استخوان پاشنه اتصال دارد. حرکات تاندون توسط یک بورس بافت همبند تسهیل می شود. این تاندون از محل اتصال به استخوان و عضله سه سر خونگیری می کند. 6-2 سانتی متر انتهای تحتانی تاندون آشیل عروق خونی ندارد و همین بخش است که در حرکات متوالی و تکراری بیشترین آسیب را می بیند.

بیومکانیک ورزشی
واحد عضله – تاندونی پشت ساق که از عضله سه سر ساقی و تاندون آشیل شکل می گیرد در بالا به زانو و در پائین به استخوان پاشنه اتصال دارند و سبب بسته شدن زانو و خم شدن به عقب کف پا می شوند.
در حین دویدن نیرویی معادل 10 برابر وزن بدن بر واحد سطح تاندون آشیل وارد می شود.

شرح حال
• آسیب تاندون آشیل اغلب در افراد مسن که گاهگاه ورزش می کنند رخ می دهد اما می تواند در ورزشکاران جوان آماده هم رخ دهد.
• باید پرسیده شود که آیا اخیراً در میزان فعالیت و سطحی که در آن ورزش می شود تغیری داشته است یا خیر.
• درد آشیل در هنگام ورزش نشانه آن است که غالباً دردی موضعی و سوزاننده است.
• با پیشرفت التهاب شروع درد با فعالیت کمتر و زودتر رخ می دهد.

معاینه
• فرد را در وضعیت خوابیده به شکم بطوریکه کف پا از تخت معاینه آویزان باشد معاینه می کنیم.
• باید محل تاندون و عضله لمس شود و هر گونه تورم- گرمی و حساسیت در لمس تشخیصی است.
• وجود کلیک ( صدای تق ) در معاینه می تواند نشانه التهاب تاندون باشد.
• وجود تحلیل عضلانی عضله سه سر ساقی که با اندازه گیری دور ساق و مقایسه با طرف مقابل انجام می شود نشانه مزمن بودن التهاب است چرا که ورزشکار بخاطر درد موجود کمتر از آن پا استفاده کرده است.
• باید دامنه حرکات فعال و غیر فعال ( توسط معاینه گر ) در مفاصل زانو و مچ پا تعیین گردد. افراد مبتلا به التهاب تاندون آشیل معمولاً‌ کاهش دامنه حرکات در مفصل مچ پا دارند.
• باید به وضعیتهای مستعد کننده آسیب هم توجه شود مثل کف پای صاف و راه رفتن بصورتی که فرد کف پا را به سمت داخلی قرار می دهد.
• باید یک معاینه عصبی – عروقی کامل در زیر زانو صورت گیرد.

علل التهاب تاندون آشیل
علل خارجی:
• فعالیت بیش از حد
• افزایش شدت فعالیت
• افزایش مدت فعالیت
• راه پله های زیاد ساختمان محل اقامت
• کفش نامناسب
• سطح نامناسب برای تمرین ( سطوح سفت )
• ورزشهای غلط کششی
علل داخلی:
• سن بالا
• تاندونهای سفت ( ژنتیکی )
• کف پا به سمت داخل در هنگام راه رفتن
• کف پای صاف
• علل پزشکی مثل دیابت, مصرف کورتون

تصویربرداری
• اولتراسوند نسبتاً ارزان – سریع و قابل تکرار است. استفاده از آن ارزیابی دینامیک حرکات و ساختار تاندون و تعیین درجه ضخیم شدگی آن را فراهم می آورد اما مشکل آن این است که اپراتور با تجربه می خواهد و وابسته به شخص است.
• MRI روش گرانی است و از سویی امکان بررسی بصورت دینامیک را فراهم نمی کند, اما در درگیری مزمن و پارگی های تاندونی بهترین روش است. با MRI می توانیم مایع – التهاب دور تاندونی – بورسیت و ضخیم شدگی تاندونی را تشخیص دهیم.
• رادیوگرافی معمولاً‌ غیر تشخیصی است ولی ممکن است تورم بافت نرم- کلسیفیکاسیون و جدا شدگی قطعه استخوانی همراه با آشیل کنده شده از استخوان را نشان دهد.

درمان
استراحت و کاهش فعالیت لازم است. کمپرس آب سرد ( یخ ) را می توان برای کاهش تورم و درد استفاده نمود. بنظر می رسد که ورزشهای کششی بهترین روش درمان باشد چرا که بهترین انعطاف پذیری را برای مچ پا ایجاد می کنند. کشش باید آهسته روی واحد عضله – تاندون اعمال گردد. هر بار برای مدت 30-20 ثانیه ادامه یابد و چند بار تکرار گردد.

پیشگیری
کلید پیشگیری از التهاب تاندون آشیل حفظ انعطاف پذیری و قدرت خوب عضلانی در ناحیه مچ پا است. گرم کردن مناسب قبل از تمرین لازم است و بهتر است با انجام حرکات کششی این امر صورت گیرد.
پیش آگهی عالی است و در صورت استراحت و توجه و انجام کارهایی که اشاره شد بهبودی حاصل می گردد.
