آزمایش NMO، مانند تظاهرات بال‌های قرمز*، یک آزمایش تشخیصی برای NMO [نرومیلیتیز چشمی] است و قطعاً برای تعیین ازدیاد آنتی‌بادی‌ها در اثر تزریق بتافرون یا هر داروی دیگری برای MS استفاده نمی‌شود. نمونه برای پردازش به کلینیک مایو ارسال می‌شود و در آزمایشگاه معمولی انجام نمی شود. ... در حال حاضر بیشتر پزشکانی که دستور آزمایش NMO را می‌دهند نمونه مایع مغزی نخاعی را نیز انجام می‌دهند، برای پیدا کردن بیماران با مایع نخاعی مثبت.
معیار تشخیصی NMO کلاسیک بسیار دقیق است، و دو معیار نیز باید موجود باشد، علاوه بردو یا سه معیار جزئی حمایتی:
1. التهاب فلج کننده چشمی
2. تخریب میلین
و در نهایت دو مورد از سه معیار حمایتی:
1. شواهدی از MRI مبنی بر یک ضایعه نخاع به هم پیوسته در 3 قطعه یا بیشتر در طو ل نخاع (تخریب میلین طولی عرضی گسترده ) ،
2. یک مرتبه MRI مغزی غیرتشخیصی برای MS**،
3. سرلوژی مثبت NMO-IgG
علاوه بر این ، درگیری سیستم عصبی مرکزی فراتر از اعصاب بینایی و نخاع با NMO باشد.
همچنین اختلالات طیفِ NMO وجود دارند که برگشت سرولوژی مثبت ON [التهاب فلج کننده بینایی]، یا برگشت سرولوژی مثبت LETM[حرکات ریز و سریع جانبی چشم] را شامل می‌شوند. که می شود گفت ، این دو سناریو می تواند بدون سرولوژی مثبت NMO IgG تشخیص داده شود ، اما معمولا چنین نیست.

یعنی باید 3 تا ضایعه توی نخاع باشه حتما یا نخاع و سر فرقی نداره؟

سارای عزیزم، این آزمایش فقط برای تشخیص قطعی Devic هستش. برای ام.اس کاربرد ندارد. برای تشخیص قطعی ام.اس از ام.آر.آی و نوار چشمی EEG و بعد مایع مغزی نخاعی بهره می‌گیرند. آنالیز مایع مغزی نخاعی که مستلزم گرفتن نمونه از مایع نخاع با استفاده از یک سرنگ درناحیه کمر (نخاع) می‌باشد از نظر وجود یک پروتئین غیر طبییعی ، قطعات کوچک میلین ، یا گلبولهای سفید خون به تشخیص کمک می کند. برا ی مثال سطوح بالای ایمونوگلوبولین جی، برای تشخیص حائز اهمیت است و می تواند شاخص پیشرفت بیماری باشد. (ایمونوگلوبولین ها زنجیره های پروتئینی هستند که بخشی از سیستم ایمنی محسوب می شوند).

برای دویک بله، فقط وجود ضایعه در نخاع است و مغز درگیر نمی‌شود بجز چیاسمای بینایی در مغز.
ولی در ام.اس در تمام جسم سفید دیده می‌شود. لطفاً تشخیص افتراقی ام.اس و دویک را در همین دانستنی‌ها ام.اس مطالعه کنید.

آزمایش مایع نخاعی حداکثر در 50% بیماران منتلا به ام اس مثبت میشه. پس میشود!
