استرس دوران کودکی موجب ابتلا به اختلالات تبدیلی مانند تشنج و بی حسی اندامی در بزرگسالی می شود

غلامرضا حاجتی امروز در بیست و هفتمین کنگره سالانه انجمن علمی روانپزشکان ایران در بیمارستان میلاد برگزار شد در جمع خبرنگاران اظهار داشت: اختلالات تبدیلی بیماری است که با علائم مغزی مانند تشنج، فلج و بی حسی اندام ها خود را نشان می دهد و معمولاً در معاینات و بررسی های معمولی نورولوژیک هیچ یافته ای از خود بروز نمی دهند.
وی افزود: به نظر می رسد این بیماری به علت تعارضات ذهن ناخودآگاه ایجاد می شود که یک قرن قبل پزشکان و روانپزشکان آن را بر اساس رفع تعارضات ذهن ناخودآگاه و استرس ها درمان می کردند.
این روانپزشک تصریح کرد: در این بیماری بیمار نسبت به ایجاد علائم آگاه نیست و می توان گفت استرس هایی که از دوران کودکی برای فرد ایجاد می شود می تواند این بیماری را ایجاد کند.
حاجتی تصریح کرد: حدود ۹ تا ۳۰ در ۱۰۰ هزار نفر در جامعه عمومی و یک سوم مراجعان کلینیک های نورولوژیک و بستری بیمارستان های اعصاب و روان به علت ابتلا به این عارضه است.
وی اضافه کرد: این بیماری زمینه ژنتیک هم دارد و باید گفت که پیشگیری خاص ندارد و فقط باید از ایجاد استرس های دوران کودکی و بزرگسالی جلوگیری کرد و به نوعی نحوه مقابله با استرس ها را از دوران کودکی به کودکان یاد داد.

استرس دوران كودكي، سيستم ايمني بدن را در بزرگسالي تضعيف مي‌كند

پژوهشگران تاكيد كردند: استرس و تنش‌هاي رواني در نخستين سالهاي زندگي و دوران كودكي، تاثيرات نامطلوب و مشكلات طولاني مدتي را در آينده براي سيستم ايمني بدن بوجود مي‌آورد.

به گزارش سرويس بهداشت و درمان ايسنا، سيت پولاك، محقق و متخصص دانشگاه ويسكونسين در مدسيون آمريكا در اين باره مي‌گويد: اين مطالعه نشان داد اشخاصي كه در كودكي مورد سوء رفتارها و سوء استفاده‌هاي فيزيكي قرار مي‌گيرند، در دوران بزرگسالي عملكرد سيستم ايمني بدن آنها دچار نقصان و نارسايي‌هاي قابل توجهي مي‌شود. نتايج اين تحقيق در نشريه دستاوردهاي آكادمي ملي علوم آمريكا منتشر شده است.

در اين پژوهش توان سيستم ايمني در بين افراد بالغي كه دوران كودكي عادي يا غيرعادي را به لحاظ استرس و تنش رواني تجربه كرده‌اند، مورد مقايسه و ارزيابي قرار گرفت. اين پژوهشگران سطح بالاي آنتي بادي‌ها را در برابر ويروس ساده نوع يك تبخال بررسي كردند.

نتايج بررسي‌ها نشان داد: افرادي كه سيستم ايمني بدن آنها سالم و قوي است مي‌توانند اين ويروس را در بدن خود كنترل كنند و به ندرت علائم ويروس مزبور در بدن اين افراد بروز مي‌كند مگر اينكه در مواردي دچار يك بيماري يا استرس شوند كه به اين ترتيب سيستم ايمني بدن آنها متاثر شده و تضعيف مي‌شود. اما در مورد افرادي كه سيستم ايمني بدنشان ضعيف است، بدن در مقابله و سركوب كردن ويروس تبخال با نام مختصر HSV-1 دچار اشكال مي‌شود و قادر نخواهد بود به طور موثر عليه ويروس فعال شده، آنتي بادي توليد كند.

تحقيقات نشان داد: گروهي از نوجوانان شركت كننده در پژوهش كه دوراني را در روماني، روسيه و چين يتيم بودند و بعد در خانواده‌هاي آمريكايي به فرزند خواندگي قبول شدند، دچار شواهد و علائمي از ضعف و نارسايي سيستم ايمني بدن هستند.

استرس در زمان کودکی و افسردگی در سن بالا

استرس مادر در دختر آثار روحی و روانی به‌جای می‌گذارد، طوری که او در سنین بالاتر در معرض آسیب‌پذیری بیشتری به بیماری افسردگی قرار می‌گیرد. بر اساس پژوهش دانشمندان، این امر در مورد کودکان پسر صدق نمی‌کند.

محققان آمریکایی در تحقیقی درازمدت به این نتیجه رسیده‌اند که اگر دخترها در دوران کودکی تحت استرس بزرگ شوند، در زمان نوجوانی و در سنین بالاتر آسیب‌پذیری بیشتری در مقابل بیماری افسردگی خواهند داشت.

این محققان که از دانشگاه ویسکانسین-مدیسون هستند، نتایج پژوهش خود را در نشریه علمی "Nature Neuroscience" منتشر کردند.

تاثیر استرس حتی در یک‌سالگی

بر اساس این تحقیق، حتی داشتن استرس در یک سالگی باعث افزایش هورمون کورتیزول در درازمدت در بدن کودک می‌شود. بدین ترتیب در مغز آن کودک پیوند بین دو ناحیه‌ی مغزی خاص تغییر می‌کند. یکی از این نواحی، مسئول احساسات و دیگری مسئول کنترل احساسات است. خانم‌هایی که پیوند این دو ناحیه‌ی مغزیشان ضعیف شده است، در دوران جوانی، در مقایسه با هم‌سن‌هایشان، بیشتر دچار افسردگی و ترس می‌شوند.

اما اینکه چرا این مساله بیشتر در دخترها دیده می‌شود هنوز کاملا مشخص نیست. احتمالا دلیل آن این است که متابولیسم دخترها نسبت به متابولیسم پسرها حساس‌تر است.

پژوهشگران برای انجام تحقیقات خود ۵۷ کودک را مورد بررسی قرار دادند. این افراد از زمان تولدشان در این تحقیق درازمدت شرکت داشتند. محققان داده‌هایی در مورد شرایط روحی این کودکان از یک‌سالگی جمع‌آوری کرده بودند. از طریق این داده‌ها، آنها اطلاعاتی در مورد روابط زناشوئی مادرهای آنها کسب کردند که آیا آنها دراین روابط تحت فشار زیاد بوده‌اند یا نه. بر اساس گفته‌ی دانشمندان، نوزادان ناخودآگاهانه متوجه مشکلات و اختلافات خانوادگی می‌شوند.

پس از آن، محققان این کودکان را در سن حدودا ۴ سالگی دوباره مورد بررسی قرار دادند و میزان هورمون کورتیزول را در بدن آنها اندازه‌گیری کردند. بر اساس این تحقیق، دخترهایی که در سنین پایین دچار استرس بودند، میزان کورتیزول بالاتری نسبت به دیگران داشتند. در مرحله‌ی بعدی محققان شرایط روحی دخترها را در سن ۱۸ سالگی از طریق یک پرسشنامه و اسکن مغزی سنجیدند. آنها به این نتیجه رسیدند که پیوند دو ناحیه‌ی مغزی در دخترهایی که تحت استرس بودند، ضعیف‌تر بوده است. اما آنها این ارتباط را در میان شرکت کنندگان پسر کشف نکردند.

این دانشمندان معتقدند که نتایج تحقیق‌شان، زنجیره‌ای بین استرس در زمان کودکی، میزان هورمون کورتیزول در بدن دخترها و تغییرات عملکرد مغزی در خانم‌های جوان را نشان می‌دهد. به این ترتیب به گفته‌ی آنان، این اولین پژوهشی است که نشان می‌دهد چرا استرس در زمان کودکی باعث افسردگی در سنین بالاتر می‌شود.

تاثیر منفی استرس خانواده بر سیستم ایمنی کودک
به گزارش روابط عمومی بیمارستان سینا: پژوهشگران سوئدی دریافته‌اند بالابودن استرس درخانواده به اختلال در دستگاه ایمنی کودک می‌انجامد.
یک گروه پژوهشی از دانشکده‌ علوم بهداشتی دانشگاه یان کوپینگ و دانشکده‌ علوم بهداشتی لینکوپینگ در سوئد طی یک بررسی دریافته‌اند، دستگاه ایمنی انسان وظیفه‌ حفاظت از انسان را در برابر باکتری‌ها و ویروس‌ها بر عهده دارد. بدن انسان برای مقابله با استرس روزمره یعنی استرس‌هایی که بسیار کوتاه طول می‌کشند، به ابزاری مجهز است. از طرف دیگر، یک موقعیت دشوار پر از استرس یا افزایش درازمدت استرس دارای تأثیر منفی بر دستگاه ایمنی است.
به گفته آنان، وقتی یک عضو نزدیک خانواده می‌میرد یا وقتی بزرگسالان خانواده در موقعیت‌های سخت و غیر تدبیرپذیری گرفتار می‌شوند، این نوع استرس درازمدت ایجاد می‌شود.
نتایج این بررسی پژوهشی نشان ‌می‌دهد، کودکانی که در خانواده‌های پر از استرس زندگی می‌کنند، دارای میزان بالای کورتیزول -نشانگر زیستی استرس - هستند.
یافته‌های محققان به ‌این نکته اشاره دارد که بالا‌بودن میزان استرس بر دستگاه ایمنی تأثیر منفی دارد؛ یعنی وقتی بدن درمعرض مقدار زیادی استرس قرار ‌می‌گیرد، دیگر تاب مقاومت نمی‌آورد. به ‌این ‌ترتیب، دستگاه ایمنی نسبت به موادی در بدن واکنش نشان می‌دهد که باید آنها را به حال خودشان بگذارد و احتمالا این امر با واکنش خودکار سیستم ایمنی ارتباط دارد.
در این بررسی، خانواده‌هایی شرکت داده شدند که کودکان پنج ساله داشتند. از والدین در مورد استرس و دشواری‌های پیش روی خانواده مانند طلاق یا بیکاری سوال شده بود. پاسخ‌ها سبب شد پژوهشگران یک گروه از کودکانی را شناسایی کنند که در خانواده‌ خود دارای میزان بالایی از استرس بودند و یک گروه از خانواده‌هایی را بشناسند که با میزان طبیعی استرس روبه رو بودند.
این گروه پژوهشی برنامه‌ریزی کرده است تا مطالب بیشتری در مورد چگونگی تأثیر استرس شدید بر بدن بیابد و این بار، کار را در افراد جوان گروه سنی 18 تا 22 سال انجام خواهد داد. این جوانان این بار خود می‌توانند تجارب منفی در زندگی روزمره و نیز تجارب منفی در دوره‌ی کودکی را گزارش کنند.
نتیجه این پژوهش در مجله Immunology منتشر شده است.
