پژوهشگران آلمانی با هدف راهی برای درمان بیماری "ام اس" کشف کردند که سلولهای دستگاه ایمنی چگونه به بافت مغزی نفوذ کرده و باعث آسیب دیدگی نورونها و بروز این بیماری می شوند.
به گزارش خبرگزاری مهر، در برخی از بیماریهای نورولوژیکی و به خصوص بیماری خود ایمنی "ام اس"، سلولهای دستگاه ایمنی در بافت مغزی نفوذ می کنند و به این بافت صدمه می زنند سالها است که دانشمندان در تلاش برای کشف این راز بودند که این سلولها چگونه می توانند از مانع هماتوآنسوفالیک عبور کنند.
مانع هماتوآنسوفالیک، لایه ای از سلولهای ویژه اندوتلیال است که رگهای خونی را که به مغز وارد می شوند می پوشاند و از طریق عملکرد ف.... کردن از رسیدن موادی که به طور بالقوه سمی هستند ممانعت می کند.
اکنون گروهی از دانشمندان موسسه ماکس پلانک برای اولین بار موفق شدند به صورت زنده حرکات این سلولهای ایمنی را ردیابی و رفتارهای مختلف آنها را کشف کنند.
مشاهده سیستماتیک این سلولها که حرکات آنها با کمک نشانگرهای با پروتئین فلورسانت سبز (GFP) ردیابی شد، امکان شناسایی مراحل گسترش سلولهای ایمنی را میسر کدو نشان داد که در بیماران مبتلا به "ام. اس" رفتار رگهایی که در دستگاه عصبی وارد می شوند کاملا تغییر می کند.
در این خصوص این محققان توضیح دادند: " زمانی که مشاهده کردیم این سلولها می توانند در طول دیواره های رگها حرکت کنند بسیار هیجان زده شدیم چراکه تاکنون هرگز چنین رفتاری در سلولهای T دیده نشده بود. این حرکت در جهت مخالف جریان خون انجام می شود. این مسیر خطرناک می تواند از چند دقیقه تا چند ساعت به طول انجامد و سرانجام، لنفوسیت پایانی این رفتار عجیب شروع به نفوذ به دیواره رگها می کند."
براساس گزارش ساینس دیلی، زمانی که لنفوسیت از مانع هماتوآنسوفالیک عبور کرد به حرکت خود ادامه می دهد و با یکی از چندین سلولهای فاگوسیت ملاقات می کند. تنها در این نقطه، سلولهای T فعال می شوند و مواد بسیار التهابی را آزاد می کنند که این مواد راه را برای عبور تعداد بیشتری از سلولهای دستگاه ایمنی باز می کند.

درمان ام.اس با متوتروکسات داخل نخاعی

مرکز تحقیقات مولتیپل اسکلروزیس و مدیریت بین المللی کنترل عملکرد مولتیپل اسکلروزیس از انتشار و چاپ نتایج مطالعه ای برای استفاده از متوتروکسات داخل نخاعی در بیماران مبتلا، در مجله نورولوژی خبر دادند.
این مطالعه از امکان استفاده از متوتروکسات داخل نخاعی در درمان بیماران با نوع پیشرونده که به درمان پاسخ نمیدهند گزارش میدهد. در این مطالعه ، بیماران از هشت نظر درمان بررسی شدند. بیمارانی که به درمان های مورد قبول FDA پاسخ نمیدادند تحت درمان با متوتروکسات داخل نخاعی قرار گرفتند و در طی مطالعه از نظر ارزیابی های نورولوژیک و معیار پایه ناتوانی بسط یافته مورد ارزیابی قرار گرفتند.
در 87 بیمار با فرم پیشرونده ثانویه بیماری ، میزان این معیار ثابت و یا با بهبود قابل توجهی نسبت به حالت پایه قبل از درمان در 89% این بیماران مشاهده شد و از 34 بیمار با فرم پیشرونده اولیه معیار فوق بدون بهبود قابل توجه نسبت به حالت پایه در 82% ثابت بود.
این درمان احتمالا یک درمان موثر بدون عوارض جانبی شدید در اشکال پیشرونده و غیر قابل درمان مولتیپل اسکلروزیس میباشد.

از آنجاییکه این اولین باری است که درمانی در فرم های پیشرفته و در فاز انتهایی مولتیپل اسکلروزیس موثر میباشد این خبر بسیار خوبی برای بیماران میباشد.
