تزریق تریامسینولون بعد از عمل جراحی زیبایی بینی

تریامسینولون هگزاستوناید یک استروئید تزریقی می باشد. هدف اصلی در تزریق استروئیدها از جمله تریامسینولون در روند شکل دادن بینی ، کاستن تورم ناخواسته می باشد. بطور کلی هر زمان که رینوپلاستی چه اولیه و چه اصلاحی انجام می گیرد همواره درجاتی از تورم بدنبال جراحی در بینی ایجاد می گردد.

میزان تورم به ضخامت پوست بینی و میزان دستکاری بافت بینی در حین عمل جراحی دارد. در اکثر بیمارانیکه عمل زیبایی بینی انجام میدهند در دو مرحله دچار تورم می شوند. در مرحله اول تورم نرم است و بعلت تجمع مایع میان بافتی در بینی ایجاد میشود و خوشبختانه ظرف 14-10 روز فروکش میکند.

مرحله دوم تورم بینی ، تورم سفت می باشد که برای ماهها و حتی چندین سال بعد از انجام عمل زیبایی اولیه یا اصلاحی بینی باقی می ماند. این شکل از تورم است که هدف تزریق استروئید می باشد.

یک سوال مطرح میشود که چرا بدنبال رینوپلاستی استروئید تزریق میکنیم؟ در برخی موارد بخاطر آنکه روند التیام را سرعت بخشیم و تکلیف شکل بینی بیمار زودتر مشخص شود ، استروئید تزریق میکنیم.

در این بیماران بافت نرم ضخیم تر مستعد تشکیل اسکار ( بافت سفت) در برخی نواحی از جمله یکی دو سانتیمتر بالاتر از نوک می باشد. اگر این روند تشکیل بافت اسکار گسترده باشد ، این بافت اسکار مانع جمع شدن پوست روی استخوان و غضروف بینی می شود و ظاهر بینی نامطلوب می شود. لذا در این بیماران استراتژی اینست که جهت جلوگیری از تشکیل بافت اسکار؛ استروئید به بافت بینی تزریق می گردد. نفع این اقدام در این بیماران آنست که شانس حصول به یک نتیجه مطلوب حداکثر می شود.

در بیمارانی که بافت اسکار قبلا" در بینی شان تشکیل شده ؛ تزریق استروئید به تحلیل رفتن بافت متراکم اسکار و بهتر شدن ظاهر بینی تاثیر گذار خواهد بود.

زمان تزریق تریامسینولون نیز بسته به نظر جراح از یک هفته بعد از عمل جراحی تا 4-3 ماه بعد از عمل متغیر می باشد و تزریقات بعدی نیز به فواصل 8-4 هفته صورت می گیرد.

یکی از عوارض تزریق تریامسینولون ایجاد فرورفتگی در محل تزریق می باشد همانطور که تریامسینولون قادر به تحلیل بردن بافت سخت اسکار می باشد ؛ توانایی تحلیل بردن بافت نرم طبیعی را نیز دارد.[تصویر: s_qoa00053f1.jpg]
