سلام
از عوارض غیر شایع (ولی خوب حیاتی)داروی فینگولیمود تاثیرش روی قلب
و همینطور شبکیه چشم هست
قبل از این که دوز اول دارو تحت نظر مصرف بشه
پزشک از سلامت این دو تا( مثلا خیلی کند نبودن ضربان قلب) می خواد مطمئن بشه
برا همین ارجاع می دن....

و من همچنان متعجبم که اینقدر مصرف این دارو در ایران زیاده! آیا واقعا تعداد کسانی که به مرحله دوم یا سوم بیماری رسیدن اینقدر زیاده یا واقعا دکترها فقط به اصرار بیماراشون که از تزریق خسته شدند رو به تجویز این دارو میارن؟! confused2

اینجا این دارو برا اینکه کسی به مرحله دوم یا سوم رسیده تجویز نمیشه که

این دارو چون برا ام اس نوع عودکننده- فروکش کننده س (RRMS) پس پزشکان معمولا برا بیمارانی که به نوع یا انواع آمپول رو بیماریشون جواب نمیده یا با آمپول حمله دارند و یا عوارض داده و .... بیمار رو روی داروی خوراکی میارن که میگن قوی تره

آره حدیث برای مرحله دوم هست فینگولیمود و تایسابری
چون داروی خوراکی که برای مرحله اول هست تو ایران ظاهرا دیفوزله اشتباه می کنم من؟دقیق نمی دونم

درسته فینگولیمد برای خط دوم درمانه و من که به دکتر گفتم ، گفتن شما هنوز تو خط اول درمانی.ولی طبق این مقاله ، در بعضی کشورها از همان اول داروهای قویتر که در ایران خط دوم هست ، تجویز میکنند .

داروهـای خـط اول

۱- اینترفرونهای بتا
- آونکس ، سینووکس
- ربیف ، رسیژن
- بتافرون ، اکستاویا ، زیفرون

۲- گلاتیرامر استات
شامل : کوپاکسون ، سینومر ، اسویمر ، کوپامر

۳- دیمتیل فومارات
شامل قرص های : تکفیدرا ، سینوتک ، دیفوزل : روزی دوبار همراه با غذا

۴- تری فلونوماید
شامل قرص اباجیو و یا تبازیو

۵- پلگریدی


داروهـای خـط دوم

۱-قرص خوراکی فینگولیمود : جیلنیا(مارلا ) ، فینگولیمد

۲- ناتالیزومب (تای سابری)

۳- الم توزومب یا لم ترادا

الان دیگه تجویز دارو بستگی به سلیقه پزشک داره
فینگولید را خط اول درمان هم شروع میکنن
قرار نیست کسی مثل من که عوارض امپول داره اذیتش میکنه صرفا به خاطر اینکه حمله نداره یا بیماریش کنترله قرص براش تجویز نشه فقط به خاطر اینکه خط دوم درمان هست

خوب منم یادمه که عوارض آونکس اذیتم میکرد ولی اون موقع چون جایگزینی نبود منو روی همون بتافرون ها نگه داشتند. اما امروزه درمان هایی مث کوپاکسون ( گلاتیرامر استات) وجود دارند که عوارض شدید بتافرون ها رو ندارند. من چند ماه پیش از طرف کوپاکسون یه کنفرانس ۳ روزه دعوت شدم و کسایی رو اونجا دیدم که سالهای طویلی از این دارو استفاده میکردند. حتی یکی بود که از سال ۱۹۹۹ و زمانی که دارو هنوز در مرحله آزمایش بوده به صورت روزانه داره مصرف میکنه و هیچیشم نیست! واقعا از لحاظ بالینی مساله ای نداشت و هنوز همون یک پلاکی که از ۱۷ سال پیش داشته رو داره!

درسته و قاعدتا باید اینطور باشه که اگر بیمار جواب نداده باشه به داروهای دیگه به این درمان رو میاره. (جواب ندادن معنیش این نیست که بیمار خسته شده باشه از آمپول زدن!)
من آمار ندارم از تعداد استفاده کننده ها از فینگولیمود در ایران اما توی این سایت میبینم که به نظر خیلی میرسند و من با توجه به جامعه آماری اینجا دارم اظهار نظر میکنم.Cool

ممنون بابت مقاله. درسته! سازمان های پزشکی در دنیا عقاید و سلایق متفاوتی برای تجویز و یا حتی پذیرش (approve) دارو دارند.
مثلا توی لیستی که آوردید نام داروی من نیست و این به این خاطره که در بعضی کشورها این دارو برای ام اس تایید نشده و تجویز نمیشه و احتمالا منبع شما هم از جایی بوده که این دارو رو در درمان ام اس استفاده نمیکردند.

پزشکا تا جایی که من می دونم می گن تا زمانی که بیماری با دارویی کنترل هست دارو نباید تغییر کنه
داروهای خط دو و سه عوارض بیشتری دارن هرچه دارو قویتر می شه عوارضش هم بیشتر می شه
اگر هم عوارض تزریق اذیت می کرده که دیگه الآن خدا رو شکر داروی خوراکی تو خط اول هم هست

نه منبع از ایرانه ، نوشته های دکتر عظیمی . منتها توی این نوشته ، فقط به داروهای خط اول و دوم اشاره شده . در جای دیگه از داروهایی مثل ریتوکسی مب و اکرلی زومب نام برده شده که نمیدونم خط سومن یا نه ولی برای عده ای این درمان استفاده میشه که قبلا برای ام اس نبود.

برای من که هنوز RRMS هستم ریتوکسیماب از ۳ سال پیش تجویز شد. به قول بچه ها چون من اون زمان شمال سوئد بودم و این دارو اونجا پیش پزشکان و مراکز تحقیقاتی توی اومئو خیلی پرطرفدار بود منو به این شیفت دادن. Wink
