این دارو در مسیر تبدیل شدن به سومین داروی خوراکی در دسترس برای بیماران ام اس است، و به طور بالقوه امن ترین و موثر ترین، دارو از نظر کارشناسان است. این دارو خوراکی، به نام Aubagio، در هفته گذشته به تصویب رسید.

ام اس عملا تنها دو دهه پیش غیرقابل درمان بود، اما امروزه اینطور نیست و داروهای " اصلاح کننده بیماری" برای بیماران در مراحل اولیه در دسترس هستند ، تعداد زیادی هم برای مراحل پیشرفته تر بیماری در حال توسعه هستند. اکثر بیماران در مراحل اولیه این بیماری، یک فرم به نام ام اس عود کننده و فروکش کننده دارند که داروهای آن تزریقی بودند.

دو مطالعه جدید، به صورت آنلاین در مجله پزشکی نیوانگلند منتشر شده است، نشان داد که داروی خوراکی BG-12، توسعه یافته توسط Biogen Idec، میزان عود در بیماران مبتلا به عود MS را در حدود 50 درصد کاهش داد. این دارو نیز به طور قابل توجهی فرکانس ضایعات مغز مرتبط با این حملات را کاهش می دهد و کند شدن روند پیشرفت بیماری در مقایسه با دارونما را باعث می شود.

مطالعات در فاز 3 به پایان رسیده است و آخرین مرحله تصویب دارو است. سازمان غذا و داروی آمریکا (FDA) نیازبه اتخاذ یک تصمیم در مورد تصویب دارو قبل از پایان سال جاری دارد.
تیموتی کوئتزی مدیر تحقیقات در انجمن ملی ام اس، می گوید این دارو به وضوح بسیار در مدیریت بیماری و کاهش ناتوانی و مشخصات ایمنی کاملا خوب به نظر می رسد.


اینترفرونهای تزریقی که اغلب در ام اس عود کننده و فروکش کننده استفاده می شود در حدود 30 درصد احتمال حمله و سرعت پیشرفت این بیماری و ناتوانی را کاهش می دهند. داروی Aubagio که به تازگی تایید شده نیز عود را در حدود 30 درصد کاهش می دهد اما با این تفاوت که یک داروی خوراکی است.

دو دارو که به طور قابل ملاحظه ای موثر تر هستند، Tysabri و Gilenya، با خطرات جدی از جمله، در موارد نادر، مرگ همراه هستند. آنها به عنوان خط دوم در درمان استفاده می شوند زمانی که رویکرد اولیه با شکست مواجه شود، در استفاده از این داروها بیماران نیاز به مراقبت ویژه دارند.

در مطالعات جدید، بیماران به طور تصادفی به دو گروه با مصرف 240 میلی گرم BG-12 دو بار در روز یا سه بار در روز تقسیم شدند. بیماران در گروه سوم دارونما دریافت کردند. نتایج این ترکیب نشان داد که این دارو میزان عود را در حدود 50 درصد کاهش می دهد. اختلاف حداقلی بین رژیم دو بار در روز و سه بار در روز وجود داشت.

خوردن BG-12 دو بار در روز تعداد ضایعات مغزی و یا به تازگی گسترش یافته را 71 درصد تا 99 درصد بسته به نوع ضایعه کاهش داد. در این مطالعه تعریف آماری معنی دار، کاهش 38 درصدی در پیشرفت ناتوانی است.

شایع ترین عوارض جانبی گرگرفتگی موقت و احساس گرم شدن و علائم گوارشی از جمله تهوع، اسهال، دل درد و استفراغ بود. اگرچه هر دو نوع عوارض جانبی شایع بود،اما پس از چند هفته اول استفاده کاهش یافت و توسط اکثر بیماران تحمل شد.
دکتر رابرت فاکس، نویسنده یکی از این دو مطالعه جدید و مدیر پزشکی مرکز Mellen برای درمان ام اس و پژوهش در کلینیک کلیولن می گوید BG-12 یک داروی ضد التهابی است که با حفاظت از اعصاب در برابر آسیب عمل می کند. این دارو یک اسید فوماریک است، بسیار شبیه به دارویی که به طور گسترده ای در آلمان برای درمان پسوریازیس استفاده می شود. سابقه ایمنی آن به خوبی شناخته شده است و به نظر می رسد بسیار قوی باشد، او همچنین گفت: "این یک روز روشن برای بیماران ام اس است، اما یک ابر خاکستری وجود دارد که ما هنوز هیچ دارویی را برای افراد مبتلا به MS پیشرونده نداریم، "

نتایج منتشر شده از دو مطالعه بالینی فاز 3 در خصوص کپسول BG-12 در مبتلایان به ام اس نوع بدتر شونده-بهبودیابنده نشان میدهد که درمان با این دارو به طور قابل ملاحظه ای تعداد حمله ها و نیز فعالیت بیماری را در MRI، کاهش داده است. در مطالعه دیگری نیز این دارو پیشرفت ناتوانی را کاهش داده است. سازمان غذا و داروی امریکا FDA) ) در حال حاضر مشغول بررسی اثرات این داروست و در صورت مثبت بودن نتایج ، مجوز ورود آن به بازار صادر خواهد شد.

مطالعات فوق نشان میدهد که دارو قادر است احتمال حملات را 50-49% و میزان پیشرفت ناتوانی را 38-34% در ام اس نوع حمله ای کاهش دهد.


اگرچه مکانیسم دقیق اثر دارو بدرستی مشخص نیست اما به نظر میرسد با مهار سلولهای ایمنی احتمالاً برای سلولهای عصبی مغز و نخاع اثر محافظتی داشته باشد.
