قرص تولترودین

تولترودین انقباضات ماهیچه های مثانه را کاهش می دهد و برای درمان مثانه بیش فعال با علائم تکرر ادرار،احساس فوری دفع ادرار،بی اختیاری ادرار استفاده می شود.

مکانیسم اثر

تولترودین بعنوان آنتاگونیست رقابتی استیل کولین بر روی گیرنده های پس گانگلیونی موسکارینی عمل می کند.هم انقباض هم ترشح مثانه با واسطه گیرنده های موسکارینی کولینرژیک انجام می گیرد. تولترودین از طرق مهار واسطه گیرنده های موسکارینی کولینرژیک سبب شل شدن عضلات صاف مثانه و دستگاه ادراری می شود.

فارماکودینامیک : تولترودین دارای اثر شناخته شده ای بر روی عملکرد مثانه می باشد.

فارماکوکینتیک : تولترودین پس از مصرف خوراکی به خوبی جذب می گردد و حداکثر غلظت سرمی بعد از 1-2 ساعت بدست می آید.تولترودین به میزان قابل توجهی به پروتئینهای پلاسما ،بطور عمده α1 گلیکوپروتئینول اسید متصل می گردد. تولترودین پس از مصرف خوراکی بطور گسترده ای در کبد متابولیزه می شود.حدود 77% تولترودین مصرفی از طریق ادرار و 17% آن از طریق مدفوع طی 7 روز دفع می گردد.نیمه عمر تولترودین 1.9-3.7 ساعت می باشد.

هشدارها :
تولترودین در موارد زیر با احتیاط فراوان مصرف گردد:

انسداد قابل توجه مسیر خروجی مثانه، اختلالات انسدادی دستگاه گوارش،کاهش حرکات دستگاه گوارش( بعنوان مثال آتونی روده)، نارسایی کبد،نارسایی کلیه،میاستنی گراویس،طولانی شدن فاصله QT مادرزادی یا اکتسابی،سالمندان،هیپرتروفی پروستات

نکات قابل توصیه

1-مصرف تولترودین در موارد بروزمشکل در تنفس،انسداد راه هوایی فوقانی ،کاهش فشار خون باید قطع گردد و بلافاصله درمان مناسب جایگزین گردد.

2-بیماران باید برای اثرات CNS آنتی کولینرژیک ،بخصوص پس از شروع درمان یا هنگام افزایش مقدار مصرف تحت مراقبت قرار بگیرند.تا زمان تثبیت عوارض CNS از رانندگی و یا کار با ماشین آلات سنگین خودداری نمایید.

3-داروی تولترودین را همراه غذا مصرف نمایید.

4-بیمار باید حین مصرف تولترودین از انجام فعالیتهای بدنی شدید در هوای گرم یا مرطوب خودداری نماید.

مقدار مصرف

مقدار مصرف معمول برای بی اختیاری ادرار: بزرگسالان:

قرص معمولی(سریع الاثر):
-مقدار اولیه: 2 mg خوراکی ، دو بار در روز
-مقدار نگهدارنده: 1 تا 2 mg خوراکی دو بار در روز،بسته به پاسخ و تحمل بیمار

کپسولهای آهسته رهش:
-مقدار اولیه 4 mg خوراکی یکبار در روز
-مقدار نگهدارنده: 2 تا 4 mg خوراکی یکبار در روز،،بسته به پاسخ و تحمل بیمار

مقدار مصرف معمول برای تکرر ادرار: بزرگسالان:

قرص معمولی(سریع الاثر):
-مقدار اولیه: 2 mg خوراکی ، دو بار در روز
-مقدار نگهدارنده: 1 تا 2 mg خوراکی دو بار در روز،بسته به پاسخ و تحمل بیمار

اتینل استرادیول , دگزامتازون , دونپیزیل , دیفن هیدرامین , زفیرلوکاست , سلکسیب , لونورژسترول , وین بلاستین , سوتالول , سایمتیدین , پنتوپرازول , اپرپیتانت , ایپراتروپیوم بروماید , اریترومایسین , سیلدنافیل , آزیترومایسین , کلوزاپین , سرترالین , بوپروپیون , گالانتامین , فلوکستین , دیلتیازم , فلووکسامین , سیکلوسپورین , وراپامیل , آتوموکستین , والسارتان , ووریکونازول , ایماتنیب , پاروکستین , آمیودارون , وارفارین , بوسنتان , دومپریدون , سیپروفلوکساسین

موارد مصرف دارو : مثانه بیش فعال

موارد منع مصرف دارو :
احتباس ادرار , تاخیر در تخلیه معده , گلوکوم زاویه باریک حاد , ازدیادحساسیت به این دارو و ترکیبات آن

عوارض جانبی دارو

خشکی دهان , سردرد , خستگی مفرط , گیجی , یبوست , دردشکمی , سوءهاضمه , اختلال دید , حالت خواب آلودگی , اضطراب , سینوزیت , اختلال در وضع ادرار( ادرار همراه با سوزش و درد) , طولانی شدن فاصله QT

من رفتم دکتر کلی بابت تکرر ادرار شاکی شدم
واسم vsol 10 نوشت روزی یکی
البته هنوز نخوردم ببینم اثرش رو

دوستان یه سوال
کسانی که دچار تکرر ادرار بودن ، با مصرف دوره ایِ قرصی که دکتر میده مشکل رفع میشه یا بلند مدت و همیشه باید دارو مصرف کرد چون این مشکل جزء سیر بیماریه ؟

من این مشکل رو اوایل بیماریم شدیدا داشتم
مدام قرص میدادن دکترا
دیگه اثر داروها بود یا چی تقریبا مشکلم رفع شده
و الان دوسه سالی میشه دیگه بابتش دارویی نمیگیرم
اما هنوزم گاها در این زمینه دچار مشکل میشم که گذراس

من تو این سالها اصلا این مشکل تکرر رو نداشتم . سرِ کار که معمولا از صبح زود تا 2 یا 3 بعد از ظهر نمی رفتم دستشویی.
ولی چند ماهه که زود به زود باید برم و شبها هم یکی دو بار بیدار میشم.نمیدونم بیماریم پیشرفت کرده یا نه. یادم رفت به دکتر بگم دفعه پیش.
تولترودین اثر داره ولی من یه بار یادم نبود یک میلیشو خریدم ، کم اثر بود.

شهرداری هم که تو شهر به این شلوغی به فکر ساختن سرویس بهداشتی تو خیلی جاها مثل مراکز خرید و بازار و... نبوده و اینم برامون شده یه مشکل.

منم دو سه ساله که دارم vsol میخورم. قبلش اوکسی بوتینین و تولترودین میخوردم ولی باهاشون مشکل داشتم

من واقعا این تکرر ادرار بیچارم کرده یعنی از مسافرت متنفرم چون تا پامو از خونه بیرون میزارم صدبرار میشه مشکلم همیشه به این خاطر میترسم برم بیرون .تولترودین هم تقریبا یه دوهفته ای مرتب خوردم هیچ اثری نداشت
